If you want my opinion, I think he's going to be Fire/Earth or Fire/Earth/Wind even though he should be Fire/Earth/Lightning.

Fire/Earth/Wind makes the most sense (Fire/Wind is genjutsu and Earth is Tree plus Foo is Earth and Torune is Wind), but they already gave us a 3-element ninja with those 3 elements (Tsuchikage). He also shouldn't be Fire/Earth/Water because of the Mizukage. I think all of the three element combos should be unique to the ninja that first had them, at least until all of the 3 element combos have been used up.

Technically yes, but not anyone does. It's like giving Rasengan to everyone because technically anyone can learn it. I'd prefer if we stick to the story, especially for something as powerful as Izanagi.

Well then you have to take Itachi off your list. He didn't use Izanagi. He used Izanami.

I think it will be Sharingan Eye + Entrance Cost (5 or 6), simply for the sake of playability. Though, since Danzo was the one who refined it into something usable beyond a Last Resort, maybe it will have expert Danzo or something.

Well the thing is Izanagi can technically be used by any Sharingan user. But to use the "complete" form you need Senju DNA. So maybe it'll be Sharingan + Entrance Cost of 5 or more and then Expert(Tree). That way any of the Sharingan guys can play it but Danzo and eventually Tobi will be able to unlock its true power.
