When I declare blockers, am I able to make a back ninja in a blocking team the head ninja before being sent out to block? The ninja's in the team are the same as they were at the end of my turn, but the order would be different.

Head Ninjas are declared when you send a team out to battle. When the team returns to the Village, there is no longer a Head Ninja. If the same team is sent out to a different battle, you would choose a Head Ninja again and it could be different from the last Ninja you chose.
