Maybe he's carrying Anko under his cloak.

It's most likely a mini-Rashomon that he has summoned under his cloak as a defense against Sasuke. But really though, Anko would be cool.

Perhaps his cloak has shoulder pads.

Or, perhaps he is just closest in the frame, and the "wide shoulders" are meant to give that sense.
