She's already getting on in years and Naruto is pretty near the point of being Hokage, so there doesn't need to be any sort of death or intermediate period. I could easily see Tsunade handing over her position to Naruto. I guess she could get killed by Kabutorochimaru, but maybe not. Maybe she'll finally show her hidden skill and kill him.

But before Naruto is ready to become Hokage, Tsunade's death will occur. She's already used Milotic Regeneration at least twice, and each uses shortens her life span. The Leaf isn't big on handing the reigns to the village to young ninjas, no matter how powerful. Namikaze was the youngest, and he was still closer to thirty.

Naruto and Sasuke with have one Final Clash with a super powered up Rasengan fulled by the chakra of his friends and his and jiraiya and everyone else in the show (like a spirit bomb), and Sasuke with a Chidori fueled by Madara and his brothers eyes. They both live and Madara is about to finish his eveil plan so they both team up to stop him (cause Sasuke turned good again after the fight for some random anime reasoning) Sasuke dies while protecting Naruto, Naruto becomes Hokage, Marries Hinata zooms out of the villiage in a peaceful scene. The the scene turns into a il****ration in a book as a hand closes the cover too the book were it will be Titled The Tale Of Naruto Uzumaki. And the person who closed the book was Jiraiya as he looks up at Young 12 year old Naruto Sleeping after a hard days training. The End
